The Add/Edit Data button from the main screen
opens the "Main Data Entry" screen:
Note that data can ONLY be modified from the
"Main Data Entry" form accessed via the "Add/Edit
Data" button on the "Start Form". Users
are free to rearrange and re-sort columns and rows in any
of the tables accessed throughout the program ("Show
Data" buttons or "View Data Tables" button).
These modifications will NOT alter the data, which can only
be changed through the "Main Data Entry" screen.
The record number indicator shows the location
of the current eye in the given sort order (as determined
by the "Sort Order" drop down box) and the total
number of eyes entered in the database. Upon initially
entering the "Main Data Entry" screen, the Record
Number will be set to "1 / 1". This will change
to reflect the correct total number of eyes upon any record
navigation.
The section at screen bottom controls navigation
through the patient records. At the top left, the total
number of entries and the current entry are displayed under
Record Number. Note that although there
may be more than one record in the database, the display will
initially always show "1/1". After any record
navigation is performed, this indicator will automatically
reset to show the correct total number of entered eyes.
Two methods to quickly navigate to a selected
treatment are provided. The drop down box at top right
will display all patients and eyes select one to move
to that entry. The Find Patient button at
bottom right will search for a specific patient name.
The "Add New Patient" button MUST
be clicked to enter any new eyes into the database.
This will open a new blank page where the boxes can be filled.
If you simply type over a previous patient's information,
the prior data will be erased and replaced with the new data.
Alternatively, to add a new eye for a patient
for whom one eye has already been entered, click the "Other
Eye" button. The appropriate data fields will be
copied to a new record, the current record indicator at top
left will increment, and blanks will be left for new data
to be filled in.

to that entry. The Find Patient button at
bottom right will search for a specific patient name.
The entries can be sorted by last name, treatment
date or in the order they were entered into SCORE. The
sort order can be controlled by the Sort Records By
drop down box at top center. This will control how the
record navigation at screen bottom decides which eye to display.
When the sort order is changed, the "Current eye/Record
Number" will also be updated to reflect where the current
record (eye) is located in the new sorting.
By default, the sort order is by order of entry.
Last Name, First Name, Birthdate, Surgeon, Refractionist,
Eye Color and Race, Pupil size in bright and dim light and
Contact Lens Wear (CL Wear) are self-explanatory. In
entering birthdate, be sure the correct year is displayed.
Typing "12/12/35" will default to "12/12/2035".
The Surgeon, Refractionist, Eye Color and Race fields only
allow entries in the drop down boxes. If another value
is desired, it must be added from the "Set Options"
menu from the start screen.

The laser settings boxes in the
operative information section and the retreatment information
section will only allow valid numbers as defined in the Set
Options Edit Lists Procedures section.
If you enter data outside this range, an error is generated
and you will be prevented from doing anything else (including
exiting). After closing the error box, enter a valid
value. If you think the range entered in Set Options
may be incorrect, hit the escape key after closing the error
box and review the ranges under Set Options Edit

The Print button will output a screen
image in landscape mode to the printer. Validate
does some simple testing for outliers. Vector analysis
will send the follow up data for one specified time interval
to the Astigmatic Analysis graph for viewing.
The "Other Eye" button switches display
to the patient's other eye, if it exists. If no data
for the other eye has been entered, it will optionally open
a new record and copy the appropriate data fields to simplify
data entry. Note that the custom field data is also
copied over and may need to be edited to insure data validity.
The "Ignore for Regression" checkbox
is used to select eyes that for whatever reason should not
be used in nomogram design. For example, if a patient
had an intraoperative complication resulting in an undercorrection
or overcorrection or the patient's outcome is deemed to be
a strong outlier, it may be desired to have the patients results
excluded from nomogram development. However, all data
is (appropriately) always used for outcomes reporting.
SCORE allows data for up to two retreatments
to be entered per eye. The add button with place a new
tab for the operative information for the retreatment to be
entered. The delete button permanently removes the retreatment
operative data for the last entered retreatment.
Follow up data following the retreatment is
entered normally under the "Follow Up" tab.
After a retreatment is entered, all follow up data will show
an indicator next to the follow up date.
